Diet for Fatty Liver Grade 1 and 2, With a 7-Day Plan โ

TL;DR - Fatty liver diet for grade 1 and grade 2
โข Who it's for: Anyone whose ultrasound came back grade 1 or grade 2, with or without raised enzymes
โข Key number: Weight loss of 3% to 5% improves liver fat, while more than 10% is generally needed to improve inflammation and fibrosis
โข The pattern: Vegetables, legumes, whole grains, fish, nuts, measured oil, 1.2 to 1.6 g protein per kg, no liquid sugar
โข Key steps: Set a weight target โก๏ธ subtract 400 to 500 calories โก๏ธ set a protein floor โก๏ธ cut three liquid items โก๏ธ recheck labs at three months
โข Safety note: Gallstones, pancreatitis, and kidney disease change the numbers, so have those signed off by your doctor
As a NutriScan nutritionist, I read a lot of ultrasound reports, and the line that scares people most is the mildest one: grade 1 fatty liver, clinical correlation advised. A 2023 meta-analysis of 92 population studies covering 9,361,716 people put the global prevalence of fatty liver disease at 30.05%, rising to 33.83% in South Asia (Younossi et al., 2023). Roughly one adult in three has some fat in the liver, and most have no symptoms.
It is common, so nobody should panic. It is also the stage where diet does almost all the work, before scarring becomes permanent. There is no drug for simple grade 1 steatosis. There is a calorie deficit, a food pattern, and time.

What Grade 1 and Grade 2 Fatty Liver Actually Mean ๐ฉบ โ
Grading comes from ultrasound, and it describes how bright your liver looks compared with your right kidney. Grade 1 means mild fat, usually with normal or slightly raised enzymes. Grade 2 means moderate fat, with the beam starting to lose the outline of the blood vessels inside the liver. Grade 3 means severe fat, where the diaphragm outline blurs too.
Grading measures fat, not damage. A grade 2 report does not tell you whether you have inflammation or scarring, which is what blood markers or a FibroScan answer. Treat the grade as a starting number, and ask for a fibrosis assessment if you have diabetes, obesity, or raised enzymes.
The naming changed too. NAFLD is now MASLD, metabolic dysfunction-associated steatotic liver disease, because the liver stores fat when the body handles energy poorly.
| Grade | What the scan shows | Weight-loss target | What to add |
|---|---|---|---|
| Grade 1 | Mild brightness, vessel walls still clear | 3% to 5% of body weight | Diet change, walking most days |
| Grade 2 | Moderate brightness, vessels partly obscured | 7% to 10%, held for 12 months | Diet, resistance training, fibrosis check |
| Grade 3 | Severe brightness, diaphragm obscured | 10% or more, specialist-led | All of the above, plus liver clinic referral |
Figure 1: The lower end of each target row above. The jump that matters is grade 1 to grade 2, where the floor goes from 3% to 7%.

Two Worked Examples of the Diet in Practice ๐ฝ๏ธ โ
Numbers help more than principles here.
Example one: a 78 kg office worker, 42, grade 1 on a routine health check. His target is 3% to 5%, or 2.3 kg to 3.9 kg. At a deficit of roughly 400 calories a day that is eight to twelve weeks. Two sweetened coffees and one soft drink come out and a 25 minute walk goes in after lunch.
Example two: a 70 kg woman, 51, grade 2 with an ALT of 68 and a fasting glucose of 108. Her target is closer to 7 kg over nine to twelve months, not six weeks. At a normal-looking BMI she needs about 1.4 g of protein per kilogram, roughly 98 g a day, so she loses fat rather than muscle. She also gets a FibroScan, because grade 2 with raised ALT and prediabetes is worth checking.
In both the deficit is modest and the biggest single win is liquid: four weekend beers and a daily fruit juice run close to 1,800 calories a week of the kind the liver handles worst. The TDEE calculator gives you maintenance, and the weight loss calculator turns a target into a weekly rate.

What the Evidence Says About Food and Liver Fat ๐ โ
Three findings shape everything below.
Weight loss is dose-dependent. The AASLD practice guidance states that weight loss of 3% to 5% improves steatosis, while greater weight loss above 10% is generally required to improve steatohepatitis and fibrosis (Rinella et al., 2023), and Mayo Clinic's patient page says the same in plainer words (Mayo Clinic, 2026).
Fructose is worse than its calories. The same guidance notes that excessive fructose consumption increases the risk of fatty liver, steatohepatitis, and advanced fibrosis independent of calorie intake (Rinella et al., 2023). Treat sweetened drinks as their own category, not as ordinary calories.
Protein can move liver fat without weight loss. Two randomized trials in people with type 2 diabetes compared an isocaloric carbohydrate-reduced high-protein diet against a conventional diabetes diet, all meals from a metabolic kitchen. At stable body weight, the higher-protein arm cut intrahepatic triglyceride by 55% more than the control (Thomsen et al., 2025).

Foods to Eat and Foods to Limit ๐ฅ โ
The practical list, a Mediterranean-shaped pattern for any kitchen.
| Eat freely | Eat in measured portions | Keep occasional |
|---|---|---|
| Non-starchy vegetables, leafy greens, spinach, broccoli | Whole grains: oats, brown rice, whole wheat roti | Sugar-sweetened drinks, fruit juice, sweet tea and coffee |
| Legumes: lentils, chickpeas, rajma, black beans | Olive oil, mustard oil, groundnut oil, measured by the spoon | White bread, pastry, biscuits, most breakfast cereals |
| Fish, especially oily fish like salmon and sardines | Nuts and seeds: walnuts, almonds, chia seeds | Deep-fried food, samosa, chips, fried chicken |
| Eggs, skinless poultry, tofu, paneer in portion | Avocado, whole fruit including blueberries | Processed meat, sausage, salami, bacon |
| Plain water, black coffee, unsweetened tea | Greek yogurt, curd, milk | Alcohol of any kind |
Everything in the left and middle columns fits in an ordinary shopping basket.
TIP
Shop from the first two columns and the third mostly sorts itself out. Food that is not in the house at 9 pm does not get eaten at 9 pm.
On alcohol the guidance is blunt: heavy drinking accelerates liver injury and fibrosis progression and should be avoided (Rinella et al., 2023). For grade 2 with any sign of inflammation, my advice is zero.
Five Tips That Move Liver Fat ๐ก โ
- Kill liquid sugar first. Soft drinks, packaged juice, sweetened coffee, and sugary chai are the highest-yield change per unit of effort, because fructose loads the liver directly.
- Anchor every meal with protein. Aim for 1.2 to 1.6 g per kilogram while you are losing weight, which protects muscle and lowers the intrahepatic fat load.
- Eat 30 g of fiber a day. Legumes, oats, vegetables, and fruit with the skin, as in this fiber-focused gut health guide.
- Measure cooking oil with a spoon. Free-pouring is where 200 to 400 hidden calories a day live in most home kitchens.
- Weigh weekly, not daily. A weekly average tells you whether the deficit is real. A daily number mostly tells you about salt and water.

Build Your Plan in Seven Steps ๐ช โ
- Write down your target weight. Grade 1: subtract 5%. Grade 2 or 3: subtract 10%. Put a date on it twelve months out, not eight weeks out.
- Find your maintenance calories, then subtract 400 to 500. A larger deficit usually fails by month three, and rapid loss is not more effective for the liver.
- Set a protein floor. Multiply your weight in kilograms by 1.2 to 1.6. That is grams per day, split across three meals.
- Rebuild the plate. Half non-starchy vegetables, a quarter protein, a quarter whole grain, plus one measured spoon of oil.
- Remove three liquid items. Name them: the evening soft drink, the two-sugar chai, the weekend beers. Vague intentions do not survive a bad week.
- Log everything for two weeks. Not forever. Two weeks of honest logging shows where the surplus is, and it is rarely where people guess.
- Recheck at three months. ALT and a lipid panel at three months, imaging at twelve. Enzymes move before the scan does, so an unchanged ultrasound at month four is not a failure.

Step 6 in practice: a two-week log, one entry per meal, is enough to find the surplus.
A Free 7-Day Fatty Liver Diet Menu ๐ โ
An example menu at roughly 1,600 to 1,800 calories, about 110 g of protein, and over 30 g of fiber. Scale portions, not the pattern.
| Day | Breakfast | Lunch | Dinner | Snack |
|---|---|---|---|---|
| Mon | Oats with milk, blueberries, walnuts | Lentil soup, mixed salad, one whole grain roll | Baked salmon, broccoli, half cup brown rice | Greek yogurt |
| Tue | Two eggs, sauteed spinach, one slice rye | Chickpea and cucumber salad, olive oil | Grilled chicken, roasted vegetables, quinoa | Apple and almonds |
| Wed | Greek yogurt, chia seeds, berries | Leftover chicken, big green salad | Rajma with brown rice, cucumber raita | Carrot sticks, hummus |
| Thu | Oats with peanut butter and banana | Tuna salad, whole grain bread | Tofu and vegetable stir fry, small rice | Boiled egg |
| Fri | Vegetable omelet, one slice rye | Lentil and vegetable khichdi, curd | Baked white fish, green beans, sweet potato | Walnuts |
| Sat | Whole wheat toast, avocado, poached egg | Minestrone with beans, side salad | Chicken or paneer curry, one roti, sabzi | Orange |
| Sun | Besan chilla with vegetables, curd | Grilled fish, roasted vegetables | Dal, sabzi, one cup brown rice, salad | Greek yogurt, berries |
Plain water through the day, no alcohol. If a day goes wrong, the next meal is the reset, not next Monday. The same plate logic tuned to blood sugar is in this blood sugar meal plan, and the shopping side in this stable blood sugar grocery list.
Indian Foods and a Grade 2 Indian Day Plan ๐ โ
The Indian version needs no imported foods and no olive oil. It needs portion discipline on rice and roti, more dal and vegetables, and fewer fried snacks.
A grade 2 lunch that costs nothing extra: dal, sabzi, two rotis, curd, salad.
On the list: all dals and legumes, curd, paneer in portion, eggs, every green sabzi, whole wheat, jowar and bajra roti, brown rice, and mustard or groundnut oil measured by the spoon. Millets are cheap and slower to raise blood glucose than polished rice. Pull back on sweetened chai, packaged juice, samosa, pakora, namkeen, biscuits, mithai, and large white rice portions. Ghee is a measured spoon, not a free pour.
A worked day for the 70 kg grade 2 example above: two besan chilla with curd, then one cup dal plus a large sabzi plus two small rotis, roasted chana in the evening, and one cup rajma with three quarters of a cup rice. That lands near 1,600 calories with about 90 g of protein. See also this list of high protein Indian foods and this Indian vegetarian plan.
Fatty Liver Plus Cholesterol, Diabetes, Gallstones, Pancreatitis, or Kidney Disease ๐ โ
Fatty liver rarely arrives alone. The core plan holds, with one adjustment each.
| Condition alongside fatty liver | What changes | Why |
|---|---|---|
| High cholesterol or high triglycerides | Swap refined carbohydrate for protein and unsaturated fat rather than cutting total fat | That arm cut triglyceride-rich lipoproteins by 33% and liver fat by 55% at stable weight (Thomsen et al., 2025) |
| Type 2 diabetes or prediabetes | Same plan, carbohydrate spread evenly across meals and paired with protein | Liver fat and insulin resistance move together |
| Gallstones | Keep the deficit modest, roughly 0.5 to 0.75 kg a week, and do not skip meals | Rapid loss and long fasting gaps raise gallstone risk |
| Pancreatitis or pancreas concerns | Lower total fat while recovering, no alcohol, smaller and more frequent meals | The one situation where a lower-fat version is right |
| Chronic kidney disease | Do not raise protein without your nephrologist, and watch potassium and phosphorus | Liver-fat protein targets can conflict with kidney-stage limits |
WARNING
Do not self-manage the last two rows. Pancreatitis and chronic kidney disease change the numbers enough that a clinician who has seen your labs should sign off before you raise protein.

โจ The everyday drink the liver guidelines actually back (about to reveal)

The One Drink the Guidelines Tell You to Have Three of โ โ
It is coffee, the only item on this page that a major liver guidance document tells you to add rather than remove. The AASLD guidance says three or more cups a day could be recommended in the absence of contraindications, based on reduced risk for fatty liver and fibrosis (Rinella et al., 2023).
A 2021 umbrella review plus meta-analysis found that among people who already have fatty liver, coffee was associated with reduced fibrosis, odds ratio 0.67 across five studies and 3,752 people (Kositamongkol et al., 2021). A separate meta-analysis of 11 studies reported a pooled risk ratio of 0.77 for fatty liver among coffee drinkers (Hayat et al., 2021).
Now the counterintuitive half. The same paper found no significant association between coffee and new fatty liver in the general population once 147,875 subjects were pooled. Coffee protects the liver you have better than it stops the problem starting.
Three cups a day, poured black. Sugar cancels the reason you are drinking it.
One condition applies: black, or with a splash of milk. Sweetened coffee puts sugar into the organ you are trying to empty, which is why it sits in the third column of the food table and black coffee in the first.
The Research Behind Protein and the Mediterranean Pattern ๐ฌ โ
Protein is the piece most home plans get wrong. In the Copenhagen trials the higher-protein arm cut intrahepatic triglyceride by 55% more than the conventional diet at matched calories and matched weight (Thomsen et al., 2025). Its split was 30% of calories from carbohydrate, 30% protein, 40% fat.
Figure 2: The composition that moved liver fat at a stable body weight. It costs nothing extra built on eggs, dal, curd, and cheap fish rather than powders.
The Mediterranean pattern has direct trial support in younger patients. A 2024 systematic review and meta-analysis of five studies in 308 children and adolescents with MASLD found it reduced ALT by 10.85 U/L and AST by 9.26 U/L, while changes in weight and lipids were small and not significant (Jamil et al., 2024). Enzymes improved before the scale did, worth remembering at your three-month blood test.
What the evidence does not support: detox teas, milk thistle instead of weight loss, gluten avoidance without celiac disease, and any plan promising reversal in two weeks.
Start With One Week, Not One Year ๐ โ
The plan on this page is boring by design, and boring is what survives a year. Grade 1 needs 3% to 5% of your body weight gone, grade 2 closer to 10% held for twelve months, on a pattern of vegetables, legumes, whole grains, fish, nuts, measured oil, enough protein, and no liquid sugar.
Pick the single change with the highest yield for your week. For most people it is the drinks. Change one thing, hold it seven days, then add the second. Two weeks of honest logging shows which one it is, and if photographing a plate is faster than typing ingredients, NutriScan is free to try with five photo scans a week. Then book the follow-up: ALT and lipids at three months, imaging at twelve.
Frequently Asked Questions โ โ
What is the appropriate diet for fatty liver?
A modest calorie deficit on a Mediterranean-style pattern: vegetables, legumes, whole grains, fish, nuts, olive oil, and enough protein, with sweetened drinks, refined starches, alcohol, and deep-fried food kept low. No single food clears liver fat. The deficit does the work, and the pattern decides how easy it is to keep.
What is the difference between a grade 1 and a grade 2 fatty liver diet?
The food list is the same. The target is not. Grade 1 usually responds to a 3% to 5% weight loss. Grade 2 and grade 3 usually need closer to 10%, held for at least a year, because inflammation and early scarring take longer to settle than fat alone (Rinella et al., 2023).
Should I follow a fat-free diet for fatty liver?
No. Liver fat comes mostly from excess calories and from sugar the liver converts into fat, not from dietary fat itself. Cutting olive oil, nuts, and fish removes the foods most associated with better liver outcomes. Cut fried food, processed meat, and pastry instead, and keep unsaturated fat in the plan.
Is a gluten-free diet good for fatty liver?
Only if you have celiac disease or a diagnosed gluten sensitivity. There is no trial evidence that removing gluten reduces liver fat, and packaged gluten-free breads and snacks are often higher in refined starch than the products they replace. Whole grains stay on the list, gluten included.
What diet does Mayo Clinic recommend for fatty liver disease?
Mayo Clinic's patient page frames treatment as weight loss through diet, smaller portions, and exercise. It states that losing 10% of body weight or more is typically recommended, while losing even 3% to 5% of starting weight can bring benefits (Mayo Clinic, 2026).
Is there an Indian diet plan for fatty liver?
Yes, and it needs no imported foods. Keep dal, rajma, chana, curd, paneer or eggs, and a large portion of sabzi. Reduce the rice or roti portion rather than removing it, and move sweetened chai, fried snacks, and sweets to occasional. A non-vegetarian structure is in this Indian plan.
Can I get this guidance in Hindi?
The plan translates directly, because it is built on foods sold everywhere in India. The Hindi terms that map onto this article are fatty liver ke liye diet, grade 2 fatty liver diet chart, and liver ki charbi kam karne wala khana. Ask for a written chart in your own language so whoever cooks at home can follow it, and if you sit most of the day, pair it with this desk job guide.
